Quartzsite School District #4 requires that you provide some documentation upon enrollment of your child. A.R.S. 15-828 is designed to help in the location of a missing child. When enrolling your child, please provide one of the following to the office:

1.) A certified copy of the pupil's birth certificate.

2.) Other reliable proof of the pupil's identity and age, including the pupil's baptismal certificate,an application for a Social Security number, or/and original school registration records and an affidavit explaining the inability to provide a copy of the birth certificate.

3.) A letter from the authorized representative of an agency having custody of the pupil certifying that the pupil has been placed in the custody of the agency as prescribed by law.

This information must be provided no later than 30 days from enrollment date.

***Additionally, the child's current immunization record must be presented at the time of enrollment, with no exception. If you have any questions please contact the school office of the school which you are enrolling your child in.
